I awoke in a safehouse, surrounded by corpses and a pile of body parts present in front of me. I quickly scavenged what I could from the area, and snuck out the front door. Unfortunately, I was immediately detected, though I couldn't tell by what due to the thick, noxious atmosphere plaguing the Mojave. I started to run, and found my pursuer close behind.
"A tunneler, at level 1? So it's going to be one of those kinds of games" I think to myself.
And I was shortly after ripped to shreds by said Tunneler and two of his buddies.
A moment later, I was back where I started. This time, I left the safehouse to the right, and was immediately set upon by two feral ghouls and a glowing one. Death ensues quickly.
Third chance, I run straight to the left, hoping that the NCR camp far to the west is still active. I run into a group of Survivors, who immediately open fire on me, one with fully automatic weaponry.
In order, I was killed by more Tunnelers multiple times, other Survivors, Cannibals, and finally made it outside of the former trading post. I say former, because it was now inhabited by a large number of cannibals with real weapons, not just crappy home-made pistols and rifles.
Then my game crashed, and I needed to head to bed.
Fallout: Dust, a New Vegas mod that turns it into a hardcore survival simulator.
I made it a little easier by not turning the native Hardcore mode on, and not including the No Second Chances mod, but I died approximately a dozen times trying different ways to get away from the Safehouse... and a handful of times outside of it for various reasons.
You literally start with nothing, and can get a jury rigged pistol with two bullets. Damage is more deadly across the board both for you and enemy NPCs (I can one shot sneak attack enemy Survivors with a Varmint Rifle, and so far I've only come across two that weren't immediately hostile), and carrying capacity is started around 45 with the base 5 strength. Said Tunnelers were completely capable of one-shotting my squishy intellectual sneaky/lockpicky/gunner character. And I haven't found any bobby pins yet even after going back north to Primm on my way to Goodsprings.
So yeah, definitely not a mod for the faint of heart. The atmosphere is creepy, especially if you add the recommended music mod, as the yellow fog permeating the Mojave makes seeing things at any distance difficult... I had a Feral Ghoul pop seemingly out of nowhere, twenty feet from my face despite carefully scanning the area (and by carefully I mean spamming the VATS key out of paranoia).
